Employee benefits are a part of the total compensation package afforded Data Connect employees. The Company provides employees with a benefit package that we believe is competitive with, or better than, others within the industry, that the company can financially support in both favorable and unfavorable economic times and that meets the most important needs of a majority of employees.
All benefits are reviewed, modified and improved as necessary to keep them current. They are subject to change or termination at any time at the sole discretion of the company.
DATA CONNECT CORPORATION - BENEFIT SUMMARY
Personal Leave – Combines sick, vacation and personal leave categories.
1st & 2nd year of service - 5 hours per pay period credited and available for immediate use up to second anniversary date (total of 6 weeks in years one & two). Leave is credited on anniversary dates after this as follows:
Second Anniversary Date 3 Weeks
Third–Fifth Anniversary Dates 4 Weeks
Sixth (and Above) Anniversary Dates 5 Weeks
Personal Leave “Cash-Out” – Opportunity to take a cash bonus annually (beginning with second anniversary date) equal to 50% of accrued leave balance (at 75% of pay rate).
Paid Holiday Leave – 9 days annually - New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, Friday after Thanksgiving, Christmas Eve, Christmas Day, New Year’s Eve.
Employee Stock Ownership Plan with 401k provisions (KSOP) - An employee will become eligible to participate in the Plan on the first entry date following the date he or she completes one year of service and has attained the age of twenty-one. General features of the plan are a 100% match by the Company on elective contributions up to 3% of annual compensation and a 50% match on elective contributions over 3% and up to 5% of annual compensation .
Tuition Reimbursement – available after one (1) year of service. Reimbursement of up to $5,250 annually in tuition costs for coursework directly related to your current job or in a logical path of promotion.
Employee Referral Program – employees receive a $1,000 referral fee if DCC hires someone they have referred for an open position.
Pay Dates – Exempt employees paid 24 times annually (semi-monthly). Paydays on the 15th and the last day of the month. If pay date falls on a weekend, paid on Friday. Non-exempt employees paid 26 times annually (bi-weekly on Fridays). Direct deposit of paychecks is mandatory.
Travel Compensation – Employees traveling out-of-town on Company business are provided additional compensation as follows:
· $100 for three or more consecutive nights & days away from their domicile
· $100 additional for a Friday, Saturday or Sunday overnight stay away from their domicile.
· If a staff member travels from his or her domicile for business on a Saturday, they will accrue an additional four (4) hours of personal leave.
Cell Phone Coverage – Data Connect has a corporate mobile phone plan with Verizon that certain staff may participate in.
The following benefits are available for full-time and part-time employees on the first of the month following thirty (30) days of continuous service (must have worked a regular schedule of at least 24 hours per week during the thirty (30) day period):
Health Insurance – Anthem Blue Cross / Blue Shield – Five (5) plan options in 2011 - DCC provides a significant contribution towards employee-only premiums. DCC provides a graduated contribution towards dependent coverage, based on years of service (generally 55%-85%).
Dental/Vision Insurance – Delta Dental PPO and Eye-Med Vision care - DCC pays 75% of employee premium. Dependent coverage offered at employee expense.
Healthcare Flexible Spending Account (Healthcare FSA) – annual max of $5,400 for 2011. Allows you to pay for eligible medical, dental & vision expenses for you and your family with a portion of your salary that is not taxed. An MBI debit card is provided for ease of use.
Dependent Care FSA – Allows you to pay daycare expenses for children under age 13 (and for care of other dependents such as elderly parents) with pre-tax dollars.
Section 132 IRS Transportation Plan – This is another “pre-tax” plan designed to save employees money on mass transportation (i.e. RTD).
Group Life/ADD Insurance – DCC pays monthly premiums for a $100,000 life and accidental death & dismemberment policy on your behalf. Employees choose beneficiaries.
Voluntary Group Life Insurance – Allows you to purchase additional life insurance for yourself and family members at very affordable rates.
Long-Term Disability Insurance – DCC pays monthly premiums for an insurance policy that enables an employee that becomes disabled and unable to continue work to receive monthly income (tax-free), of 60% of their pre-disability earnings, up to $5,000 per month.
